Download the blank form, complete it by hand, and mail it to the same address; or

Request a paper copy of it and our Volunteer Handbook, by calling 434-218-3727 or sending a request to cvillevillage@gmail.com

We ask that you read the Volunteer Handbook for an introduction to how the Village works, BEFORE applying. It's available by clicking here or as a paper copy on request, as noted above.
